Give your customers a simple public form to enter service complaints. Includes a "Complaints" form with fields for:

  • Name
  • Phone Number
  • Incident Date
  • Incident Details
  • Suggestions
  • Incident Location

If your business does not have more than one location, you can delete the Incident Location field from the Form Editor.
